The problem
Three branches ran three different systems. A patient seen in two of them had two histories, and pulling a report for an inspection took days of manual work.
What we did

Wrote a migration that reconciled duplicate patients by document number, with a manual review queue for anything ambiguous.

Designed the record around the visit, so every prescription and payment is attached to a specific consultation.

Put every read and write of a record into an audit log from day one.
Where it landed
One patient, one record, across branches. Inspection reports are generated in minutes, and the audit log answers who saw what.
